Have the policy details ready
Collect the named insured and address, each insurer's name and NAIC number, policy numbers and effective dates, limits for each line of coverage, and the certificate holder's name and address. Note any endorsements the holder asked for, such as additional insured or waiver of subrogation.

Certificate doesn't change the policy
An ACORD 25 only summarizes coverage. Listing a holder as additional insured on the certificate doesn't add them unless the policy has that endorsement.
Expired or mismatched dates
Effective and expiration dates must match the policies in force. Holders often reject certificates that expire before the contract ends.
Limits below the contract's requirement
Compare each limit with what the contract asks for, including per-occurrence and aggregate limits and umbrella coverage.
Wrong certificate holder
The holder's legal name and address should match the contract exactly. A wrong entity means a new certificate.
Missing insurer letters
Each line of coverage must point to the insurer (A to F) that writes it, with that insurer's NAIC number.
Unclear description of operations
Holders often need a specific project, location or contract number in the description box. Vague wording leads to requests for a new certificate.
